PARENTS NIGHT OUT — PARTICIPANT WAIVER & RELEASE OF LIABILITY

By checking the box below, I acknowledge and agree to the following:

1. I give permission for my child(ren) listed above to participate in Parents Night Out.

2. I understand that Parents Night Out is a supervised event and that staff will be present at all times.

3. I agree to notify staff of any medical conditions, allergies, or special needs my child(ren) may have.

4. In the event of a medical emergency, I authorize staff to seek emergency medical care for my child(ren) if I cannot be reached.

5. I release and hold harmless the organizers, volunteers, and venue from any liability for injuries or losses that may occur during the event, except in cases of gross negligence.

6. I understand that registration fees are non-refundable unless the event is cancelled by the organizers.

7. I confirm that all information submitted in this form is accurate.
